WHY SHOULD I GIVE?

As our community continues to grow, so does the need for quality health care. Recognizing that the best health care is the care delivered closest to home, Carroll Health Foundation was established as a means of support to St. John's Hospital - Berryville. Thanks to a team of dedicated professionals at St. John's Hospital - Berryville, lives are saved and patients are able to recover in the comfort of their own community.

As the cost of technology continues to rise, Carroll Health Foundation, through excellent stewardship of the donations received, is continually positioning itself to supply the hospital with needed equipment designed to enhance the hospital services. These are items that the hospital budget couldn't afford otherwise.

St. John's Hospital - Berryville is not supported by tax dollars and your tax deductible contribution will help keep the hospital on the leading edge of technology while providing this community and region the best in health care.

Carroll Health Foundation is a non-profit supporting organization of St. John's Hospital - Berryville (SJHB). SJHB is a private, non-profit hospital accredited by the Joint Commission on Accreditation of Healthcare Organizations and is affiliated with St. John's Health System-Springfield, MO.

HONORING OR MEMORIALIZING A LOVED ONE

Foundation Friends

Carroll Health Foundation accepts donations in honor or in memory of a loved one. Upon receipt of a donation, Carroll Health Foundation sends an acknowledgment and tax receipt to the donor and also notifies the family that a donation has been made in their honor or memory. Many families request donations be made to the Foundation in lieu of flowers, cards, or gifts. Donations are considered tax deductible and may be made in any dollar amount. All donors are recognized in the Foundation Life newsletter.

Foundation Friends

General donations may be made to Carroll Health Foundation in any dollar amount and are placed in the Foundation's general fund for the purchase of added technology designed to enhance the services already being provided by St. John's Hospital - Berryville. However, if you have a specific project, department, or equipment you would like your contribution designated to, just inform us of your wishes. All donors are recognized in the Foundation Life newsletter.

TREE OF LIFE CLUB

A Tree of Life was created by Carroll Health Foundation as a way of publicly recognizing and honoring contributors to the Foundation. Located beside the gift shop in the hospital's main lobby, it is comprised of leaves, apples, and stones which can be permanently engraved with your special message.

The Tree of Life is an excellent way to honor or remember a loved one for generations to come, not to mention the tax benefits it provides. Donors to the Tree of Life are also recognized in the Foundation Life newsletter.

GOLDEN TREE OF LIFE

A Golden Tree of Life was created by Carroll Health Foundation as a way of publicly recognizing and honoring larger contributors to the Foundation. Located just inside the south visitor entrance, it is comprised of golden leaves, golden apples, and golden stones which can be permanently engraved with a special message.

THE LEGACY CLUB

To become a member of the Legacy Club at Carroll Health Foundation, simply let us know that you have included St. John's Hospital - Berryville in your estate plan or intend to do so by providing us with a Statement of Intent. There is no minimum gift value required to join, and it is not necessary for you to disclose the amount of your future gift, although you are free to do so. All Legacy Club members will be publicly recognized on the donor wall located in the main lobby along with being recognized in the Foundation Life newsletter.
